Congrats!!! That sounds a lot like my birth, you poor mama!! 
46 hours total, 5cm to 10cm and pushing baby out in 27 minutes... and an epidural that didn't work. I wonder if I had one of those problems you listed? I'm still baffled as to why I had no pain relief.
