Was he standing on her front door step when she said that? If so, that was it for them. He asked Dr. Nolan for an overnight pass, he goes to Lydia's house (very sweet, traditional looking home, oozes domesticity, totally can't see House living there), her little boy answers the door. Lydia shoos the boy away, she explains they'd wanted to move to Arizona years ago, for her husband's job, but were tied here because of dh's sister/her friend. So now sister is better they're finally leaving.
Next scene Nolan is getting into his car and there's House sitting on the curb waiting for him. He tells Nolan how miserable he feels and Nolan praises him for the progress he's made. House put his heart out there, got hurt, and instead of retreating into seclusion, drugs and alcohol, he came to him, Nolan, and told him about his pain.
And House gets his own Re-Birthday party, acts snarky towards Alvie, and smashes the cake.
