Ohio exemptions... how does this work?

So, I'm getting ready to apply my DS for pre-school and they 'require' 'vaccine records'. Which, since he's never been vaxed, we don't have So... what do I take in instead? Is there something I can print off online for his/our philosophical exemption? Do they have something that we sign there (doubtful)?? How does this work, exactly?

It looks like you just submit a written statement to the school. Be sure in your letter to site the law above.

I haven't had to use it yet, myself....but my local school district has a down-loadable vaccine exemption form right on the web sight.(there's a line to check for religious/philosophical) I keep a copy handy, just in case. I'm assuming a public preschool would accept the same form. It is my understanding that you DO NOT have to elaborate on the objection. Simply checking the appropriate box is sufficient.

Thanks for everybodys recommendation. After a bit of googling I found and used this form:http://rvpi.com/Forms/Ohio%20forms/o...0exemption.pdf I just handed it in with all the info, and they didn't say a word
