I need to write an exemption letter for my oldest DD, she is in the middle school now and I haven't had to write one for that school nurse yet. (I got a letter stating if she didn't get her tetanus booster she would not be allowed back in school after 10/5). I totally forget the one I wrote for my other children and need help. It is a religious exemption and its for the state of MA, the one I wrote before was very short and to the point but now I can't remember and can't find it. Thanks for your help, its greatly appreciated.

The the practice of vaccination conflicts with my sincere religious beliefs. As such, (name) will not be vaccinated.

In accordance with MA code ALM GL ch. 76, § 15 (2007):

Quote:
In the absence of an emergency or epidemic of disease declared by the department of public health, no child whose parent or guardian states in writing that vaccination or immunization conflicts with his sincere religious beliefs shall be required to present said physician's certificate in order to be admitted to school.
Sincerely,

parent name

To Whom It May Concern:

or Dear Mrs. so and so:

(We / I) {First and Last name(s)}, as the {(parent (s) / guardian(s)} of ______________________(name of child) are exercising (our/my) rights under the US Constitution and M.G.L. c. 76, ss 15C;, Mass. Acts, Chap. 111 § 10A, and Statute: Sec. 67F to receive Religious Exemption from Vaccination, ALL injections, & testing.

Applicable law has been interpreted to mean that a religious belief is subject to protection even though no religious group espouses such beliefs or the fact that the religious group to which the individual professes to belong may not advocate or require such belief. Title VII of the Civil Rights Act of 1964 as amended Nov. 1, 1980; Part 1605.1-Guidelines on Discrimination Because of Religion.

Our legal rights are guaranteed by the free exercise clause of the First Amendment to the U.S. Constitution. Recent court decisions have upheld the rights of individuals seeking exemptions from immunizations based upon personal and religious reasons. On the U.S. Supreme Court level in Frazee V. Illinois Dept. of Security, 489 U.S. 829, it was found that a state may not deny an exemption simply because a person is not a member of a formal religious organization.

Please keep this Vaccine exemption in my child's school file. Thank you.

Sincerely,

Your and your dh name
Get it notarized and hand it to the school nurse.
