In our baby class for our first, it was recommended to carry around a 'baby' for the dogs' benefit. I thought this may be good for DS to see mama with the pouch on even if he knows it's only his baby doll. He's 20 months old so I'm not sure how much baby talk he understands. What do you think? Thanks!

I don't think it can hurt, but it might be a bit confusing to him -- I think he is probably old enough to understand that that's not the a REAL baby and obviously won't cry, nurse, etc. It might be a bit confusing to go from the fake baby to the real baby when the time comes.
I wouldn't underestimate how much your ds will understand -- and there are lots of great AP books about a new baby joining the family. Dr. Sears has some. Kids can understand a LOT before they can speak or articulate their thoughts.
You could get him his own doll carrier to carry around his baby doll, and talk about what he can do with his baby like you will do with your baby. But the doll can be his to diaper, hold, sing to, etc. KWIM? I think that might be less confusing than your carrying the doll around.

My last two are about the same age gap as yours will be. You'll be surprised with how much your little one understands. And to be honest, it didn't take long at all for him to get that the baby was ours. I think he'll probably just wonder why you are carrying a doll around.
I'd get him a baby carrier and talk about the baby in you and you'll be just fine.
