has anyone been SUCCESSFUL refusing retraction???

My 5-week old has been diagnosed with a ureterocele that needs to be incised, and the surgery involved will be passing a scope and laser into his bladder. They are also recommending tests which involve a catheter. The NP that we saw (who was a total pro-circ UAV, but that's a whole different story) was adamant that they would have to "loosen" the skin around the opening, use a steroid cream, and then lyse any adhesions. She refused to admit that this was in fact RETRACTING. And she kept insisting that they would NOT be retracting his foreskin in any way. She was a Urology specialist, btw. Not to mention that she did pull back on his foreskin during the exam, though only slightly.

We are meeting with the actual Urologist on Monday and I want to be prepared. I am hoping he'll be a little more...informed. I definitely plan on being very insistent that NO retraction is to be done for any of the procedures, but I am worried that my concerns will be poo-poo'ed or blown off because HE is the doctor and HE knows better than I. I am under the impression that none of the procedures that need to be done actually require any retraction, but this is NOT what the NP seemed to be saying. I am worried that since he is in the same practice as she, he will have the same philosophy. I can only demand so much of someone who's been taught the exact opposite, yk? And unfortunately these people are like the ONLY pediatric urologists in the entire area. This guy comes highly recommended, but I have no idea if he is actually foreskin-friendly.

So, how likely is it that my requests will be honored (even if I come in stacked with research and stuff) if I flat out refuse his foreskin to be manipulated? What happens if they say that they cannot do the procedures without "loosening" the skin? I don't really have many other options as far as other practitioners!

Write out a document specifying exactly what you will and will not permit, and make him sign it before you agree to let him do anything.

Tell him up front that you will sue if he violates the agreement.

I wouldn't hesitate hiring a lawyer to write up a letter specifying what exactly retraction, lysing etc. are and that legal action will be coming their way if anyone attempts any manipulations.

Is the NP with the urologist's practice, or with your pediatrician? Retraction and breaking the adhesions is unnecessary for inserting a catheter, so if she is with the pediatrician I wouldn't worry about what she says. However, if she is with the urologist, she may be involved in doing the procedure, and probably does it the same way the urologist (her boss) does; so I would be very worried.

Is you location line current? I would be surprised if there weren't other pediatric urologist fairly near by. There certainly should be many in NYC, which is just a few hours drive.

I think the chances that your clearly expressed wishes will be completely ignored are very high.

I strongly urge you to phone Marilyn Milos by phone and speak with her. She will have very good advice for you. She is a registered nurse and the founder of NOCIRC. She is a lovely lady! She will be able to assist you in finding good information and care for your child.

Ditto this. I think the odds are extremely good that the NP (who doesn't perform the procedures, I assume) is parroting what she's seen and heard from the urologist.

Unless your son's condition is urgent, I would strongly advise you to find a different practice, even if you have to travel quite a ways to do it. We have had more than one story on here of intact boys going in for surgery and being completely retracted with adhesions broken -- with serious consequences. This is not something that can be taken lightly.

I am sorry you have to deal with this, but forewarned is forearmed. It's just too bad we have to fight medical ignorance on this front.
