I'm laughing over the cheeseburger/pizza analogy.
With my last hospital birth, the only people involved were me, my DH, my one nurse (luckily I showed up right at the beginning of her shift and she stayed with me until the delivery), and the OB (who showed up when I started pushing). At the end there were a few more nurses to help, but they didn't really register on my radar screen.
I didn't really have to fight anything, probably because I had an uncomplicated birth. My nurse did accommodate me because I couldn't stand being on my back so when it was time for the monitoring (supposed to be 15 minutes every hour) she just held the monitor against my tummy while I was on my hands and knees and got 2 strips. As long as those two were good she let it go. I had agreed to a heplock rather than an IV with my OB, but for whatever reason no one ever did it so I didn't have to have either.
