I found one study that addressed diaphragm usage without spermicide vs. using it:

The 12-month cumulative pregnancy rate per 100 women for consistent use (defined as no unprotected intercourse and use of only assigned method) for diaphragm-without-spermicide was 19.3 (95% CI, 7.5–31.1) and diaphragm-with-spermicide was 12.3 (95% CI, 2.8–21.8)

I went to get sized at the OB and the ring she thought was the size I needed was all cracked and brittle. She bothered to tell me "we don't prescribe these much anymore". I'm surprised she even knew what to do! The pharmacy had to order it, pharmacist made a comment about how rarely they handled them. Between all that and the lower effective rates, I feel behind the times, but the diaphragm or condoms are all I am really comfortable with, and DH doesn't like condoms.
