Once a month, we have my dh's favorite Old El Paso style tacos. 
On Friday nights, we usually have a more kiddie menu. Tacos, homemade pizza, hot dogs, chicken fingers, etc. "Fun" food.
On Saturday mornings, I make a big, cooked breakfast. Eggs, grits, sometimes meat, biscuits.
On Saturday nights, we have an at-home date night. I feed the kids early, put them to bed, and then we eat just the two of us. This runs the gamunt, with the only requirement that it is "good".
Anything from grilled steaks to nachos to barbecued shrimp to take-out. Sometimes it's themed, sometimes, it's just good.
On Sundays, I have an easy breakfast (premade bakery treats, bagels, banana bread, cereal, etc).
Sunday lunch is soup.
Sunday supper is breakfast for supper.
The other 4 days of the week are for whatever. I try to have rice only a couple of times (I love rice, and I could eat it daily; dh not so much). I try to eat vegetarian or low meat once a week. I try to vary the meats and sides somewhat. At least once a week, I try something new (something I love to do).
This is tweaked for our lifestyle. I'm a SAHM with 2 kids that still nap daily. I can easily make supper during their nap time, so my weekdays are time for longer cooking things. Weekends are family time, and I don't want to spend as much time in the kitchen. Sundays are particularly go-go-go (in comparison to the rest of our week) because of church. It's hard to get everyone up and out quickly, and at lunch time, everyone is starving and tired, and the soup meets our needs the best. We get home around 12:30 or 1:00; normal lunch time is about 12, and normal nap time is around 12:30. This way, we can eat quickly and get to naptime quickly, too. It's even better for our needs than eating out (which inevitably leads to a melt down waiting for the food).

I've tried to get into doing this, but I can never stick with it because our lifestyle is so unpredictable (never know if we're going to be home on a certain day, etc.). We do try to do Taco Tuesdays & pizza on Fridays, and we usually go out for dinner on Saturday nights, but even those we don't stick with every week.

I do some serious meal-planning largely because I'm expecting #3 momentarily and I'm a little freaked out about shopping and cooking with three kids. I get my groceries delivered now, so I need to be organised. It's also the only way I can keep DH from adding meat to every meal!
Our weekly schedule (just for dinner) is:
Monday: Seafood (scallops, shrimp, mussels, etc)
Tuesday: Poultry
Wednesday: Pasta (vegetarian)
Thursday: Vegetarian (lately it's been meat....midwife-ordered!)
Friday: Fish
Saturday: Vegetarian
Sunday: Meat...we have a big Sunday dinner mid-afternoon after church....it's like a holiday every week and really brings us together.
I also like how this structure makes me look hard for new recipes in each category. I think we eat a much more diverse cuisine this way. I try to also take a big-picture view of our diet each week, which is easier if I'm planning....so if we have a cheesy, saucy dish on Wednesday, I make sure we have lots of leafy greens and something like beets on Thursday.

I generally make pancakes for breakfast on Saturday mornings.
Like others have mentioned lots of it is work/schedule related. Right now my schedule has changed and I have not gotten a good rhythm going.
This thread has made me think about getting back into some routines again.
I remember an article in Mothering years ago about set breakfasts.
Monday muffins, Tuesday polenta, Wednesday oatmeal etc...
We def. will never have that much variety for breakfast
Living in Japan means we eat rice for breakfast most days. Although I have come to realize DD1 does not like rice for breakfast so much. So I have been trying to bake more bread and freezing it.
