I think it depends on the style and exactly what you want. With DS (2nd child), I used the one my midwife brought, my bottom was free-hanging and I don't think my feet were on the ground (it was one of the bent aluminum pipe types), but I don't think they were meant to be. I'd started having the urge to push but couldn't find a comfortable, effective position, and after I got myself onto the birthing stool, it just felt right, and DS came out fast. No tears or anything, and after my long labor, it was nice to not have to push for long at all.
I can't imagine a stool that I rested my bottom on doing the same thing, but I also can't imagine laboring on this at all, it was very helpful for pushing, but even just sitting on a chair would've been far preferable for labor (though I preferred standing or our tub).
