What I do with this form from WIC?

So WIC gave me this documentation of birth form to be completed by a medical representative (nutritionist gave me the form and said have my ob/midwife fill it out, but all it asks for on it is a medical rep) after my baby is born. Well, problem is I'm planning a UC so there is no medical representative. Do you think they'd accept a pedi filling it out? Not sure what to do about it and really, I don't want to have to ask them that and admit I'm UCing.

hmm, is this proof of pregnancy or proof of birth? with the proof of pregnancy i imagine a free clinic would be able to complete it, no problem.

maybe a midwife would be willing to do it for a small fee, or a favor? Good luck!

It's just a proof of birth. All it asks for from the medical rep is length, weight, and weeks gestation of baby at birth. Then it asks for a signature and title. I guess I could try to find a midwife to fill it out for me. Not many are close, but I guess I could try to arrange for one for a postpartum visit.

For me, when I deal with WIC, I never worry about those forms. I just give them a copy of the BC when it comes in. I would think that that particular form (and the need to have it done immediately rather than wait for the BC to arrive) would be solely for the benefit of getting formula ASAP.

As for who to fill it out, your pedi can fill it out (actually, I would fill it out and then just ask them to sign it when I took the baby for a check up). I usually get something similar to that as my "Proof of live birth" from my pedi for each birth, which I then send in for my BC.

In the past they were okay with bassinet cards. When I UC'd I just made an index card with my baby's stats and it was good enough for them.

If it were me I'd fill it out and have my DH sign it. If she said you could have your midwife sign it then your husband is just as good as he'd be the one "helping" with the birth and witnessing it. I'd make his title "Midwife" since that is pretty much what he is, a lay midwife.

That's just what I would do.

Oh, didn't even think about this part yet... WIC paperwork is such a pain. Let us know what you do and how it goes. What state are you in?

WV, but I think a PP was right that it's mostly for getting formula right away since it does ask that question. Since I'm going to be BF I don't think I'll need to worry about it so much.

For DD2's birth certificate we actually got WIC to give us that info they are asking you for. We had gone in just for curiosity sakes (here the WIC office is in the district health department) and they weighed and measured her. Turned out to be useful since a week or so later we needed something like that.

I'd imagine a ped. would be fine. Our ped. would be the one to do our pp check (family dr.) so I don't see how it would be too much different from a midwife or ob being the one doing it.
