HELP! Miserable baby. Diaper rash. UTI?

Hey...my baby feels rotten. He has a rash on his bottom for the 4th day now. We've been letting him go bare butt and putting Bourdeaux's butt paste on it with no improvement. There are little tiny bumps in the rash as well as red skin. I think he has some bacterial or yeast or fungal overgrowth or something. He's getting fussier and fussier. Also I think it's starting to hurt him to go to the bathroom because he's been crying after peeing, which makes me wonder if he has a UTI or something. I don't know how to comfort him and it doesn't seem to be improving.

At what point to I take him to the doctor?

I always err on the side of going to the doctor if I'm not sure.

My thought is that if he cries while peeing, it could be a uti, and if he cries after peeing, it's probably just that it stings his rash. I'm no expert, though.

How old is your baby? Any change in foods? What kind of diapers? If cloth, what is your washing routine like? Sometimes diaper build-up can cause rashes. Certain foods to it to my ds1, like soy. Can be lots of things. But if I think there is an infection, I would go to the doctor.
