I work at a clinic and was at a meeting just a little bit ago with an OB and a bunch of OB nurses talking about the current procedure for ordering ultrasounds. The topic of 20-week ultrasound came up and what to do if the patient doesn't have a regular OB, etc. I quietly raised my hand and said I had a midwife and about half the room GASPED in unison! I had to bite my lip to keep from laughing out loud! I just threw their world into a little tailspin for a few seconds 
On the other hand, one of the scheduling girls wanted to talk to me afterwards because her daughter is using a midwife this time around and she had some questions!
