Medical Power of Attorney for Childcare Giver

Just wondering how many have some kind of letter or power of attorney in place for when your child is in someone elses care.

It just occured to me that if something happened to DD when I am at school, what would my friend do? I have looked for sample letters and can't find one.

Anyone have one? Is it really necessary? I would love to see a sample one.

For other child care's there is just a "release" which authorizes the provider to seek medical care if I cannot be reached. If this is what you mean, you could even create such a form on your computer. Your friend could carry it with her in the event she needs to take your child to the hospital or something. It would say something like "I authorize________ to seek medical care for my dd_________________in the event I am unavailable." with your signature. I don't think it would be enough legally for something like surgery but it would be enough to at least get them in the door at the ER to get treatment started. Good luck!
