Crampy contractions (13w), no bleeding, what to do?

little background: My baby has a giant omphalocele (abdominal organs outside the abdomen) and may have other abnormalities/genetic defects/disorders. We have been told there is, at this point, about a 50% chance of making it to birth. There's about a 30% chance of trisomy 13 or 18, but we won't terminate, regardless.

So, I'm 13w3d, officially fully in the 2nd trimester. For a couple of weeks, I've had on and off contractiness, feeling like really long braxton hicks. For the last week, they've been getting more painful, like cramps plus very, very contract-y. My uterus is very hard during them. I went to the er last saturday at the advice of my midwife, but they concentrated more on my bp, which was inexplicably high for a couple of hours, but went down on its own. I was also spilling protein (2+) in my urine, but I don't know that's significant to this.

They told me not to lift anything. I've been trying really hard to follow that. I've been taking it very easy, with the exception of a couple of last-minute shopping trips, and Christmas with the family (though I sat for nearly all of that).

I spotted a tiny bit last night, but nothing since.

I have an appointment with the perinatologist to talk about the protein and bp, and for the cvs, on Tuesday. I've thought about not getting the cvs, since I'm contracting so much as it is. I'm taking my bp a couple of times a day, and it hasn't gone back up into the worrying range.

Do I do anything? Or just wait and see? And if I do start bleeding, do I have to go in to the er, or can we wait until Tuesday's appointment? I know there's not much to be done for a miscarriage, and my going to the hospital would freak Cora out, so if I don't have to, I'd rather not.

If I miscarry at home, do I save the POC for the genetic testing?

I am very sorry you are dealing with all this. I would try to stay well hydrated and lay down as much as possible. If you start to bleed, perhaps going to the ER will be best since you are 13 weeks. They will most likely confirm you are miscarrying and offer a D&E, at that point you can choose whether you wish to miscarry naturally.

Also, if you don't plan to terminate, then why do CVS? I think since you are contracting, that test can make things a lot worse or add complications. Can you wait and do a Amnio later?

I second PP, rest and stay well hydrated. Go to the ER/call midwife if you start bleeding (or just feel like the contractions/crampiness is too much/worse/etc.)

I also vote against doing CVS, since it does come with a risk of miscarriage.

Congrats on your new babe and for accepting the challenge of this pregnancy into your life. Stay strong!

I've had 4 miscarriages, all at home except the first one and I also had a baby who died just after her birth at 33 weeks, due to a fatal abnormality. So, my advise would be to be in contact with your dr or midwife, and stay hydrated and well-nourished.

If you begin to bleed, you don't need to go to the ER, and unless it just makes you feel more comfortable to do so, I wouldn't really recommend it. Now, if you are bleeding very heavily (like filling a pad in less than an hour) you might want to go in. You def. don't want to hemorrage at home.

With the prognosis for your little one, you probably won't make it to term, so you need to get some support for that. If you miscarry, your body will probably do a great job of doing what is necessary. You can have your baby miscarry at home, then go in for an ultrasound to confirm that there isn't any retained placenta. You can even bury your baby should you choose to. I had a miscarriage at 12.5 weeks and we buried her under a tree in our backyard. That was deeply fulfilling.
