Newly sore nips at 3 wks

Hi all -

My LO is 3 wks old today and we've had our share of challenges - he lost 12% of his body weight 3 days after birth despite constant nursing. Because of this, we were put on a pump + supplement regime which I am now trying to get off of and back to EBF.

Here's my question: in the last 2 days I have developed sore nips for the first time since his birth. The actual skin on the nipples seems fine and not abraded - no chapped skin, definitely no bleeding or whatever - but the whole operation is ultra sensitive, reminiscent of how they felt when I was first pregnant.

My LC has complimented our "strong latching skills" - DS does like to work his way to the end of the nip despite a good initial latch, but I take him off and re-latch unless it's the end of the feeding and he's already dozing off / not doing any nutritive sucking.

I was wondering if y'all had any thoughts about what might be causing this. I switched to a hospital grade pump (the Medela Symphony) a week ago and I experience some discomfort while pumping. I also switched DS's supplementation from finger feeding to bottles (First Years Breastflow) on the advice of the LC a week ago.

Thoughts about the reason for the soreness and how to reduce the discomfort?

Did you know you can order flanges made specifically for the size of your nipples? Maybe your flanges are too small for your nips?

The other thing that I know nothing about but have heard about when it comes to nipple pain is vasoconstriction. You might want to google that and see if that may be a culprit??
