Need help getting birth certificate

My CNM basically abandoned me after my homebirth and has not followed through on multiple requests for my baby's birth certificate. He's 7 1/2 months now. I really want the BC so I can get his SS card so we can do our 2009 taxes. If I can't get her to respond, how do I get a BC? I'm in IL if it makes any difference.

Did she fill out a state birth registry? If so, you should be able to apply for it from the state. If not, I would tell her if she doesn't get the BC to you within 1 week (or whatever seems reasonable to you at this point) that you will be reporting her to the state nursing board and filing a formal complaint.

Has she not responded to you at all? I know my midwife can't get me a copy of the BC, but does have to file the birth with state records. Did you ever sign a form that had the baby's legal name on it and birth stats (that's what the birth record paper would have looked like)?

i dont know the laws for your state but you might check into how one goes about getting a BC if they had a UC... here they send you a big packet to fill out with things like proof of pregnancy, and residency and then you can get a BC

You can contact your state vital records department and see if she filed records of the birth. If she did then you can order a copy of it from them.

I would first check with your states vital statistics office and see if she perhaps sent the info in. If she has then you would be able to obtain a copy. However, if she did not report it to the state I would send her a certified letter stating that you are requesting this information to be sent to the appropriate state agency so that you may obtain your childs information. I would also check with the agency to see what she would need to do to be able to get it registered so that if she responds to you at least have some idea of what needs to happen.

Thanks for the info. She has had some personal issues and is just flat out ignoring my requests. She did not fill out anything or have me sign anything at birth.

I had a big nightmare. But it was with my town (incompetent fools). My MW had to fill out a long homebirth form first though.

Also - since I had waited until she was over a year, then I got to deal with the state instead of the town.

can you just register the birth yourself and say there was no midwife? we can do that here in MD, in fact, that's exactly what we go.
