3yr old and potty accidents

DS1 is a little over 3yrs old, we started with EC'ing and he has been using the toilet exclusively for at least a year. There have been no major changes in our life lately but he keeps having "accidents" and I'm finding it harder and harder to keep my cool because he KNOWS better. He has only had one poop accident in the last year and very very few pee accidents. On the weekend he pooped in his underwear when he was supposed to be napping and made a mess in his room.
Yesterday he did it again and smeared it all over his brother, his brother's crib, his dresser, the floor...it was awful.
Today he pooped in his underwear in the morning and again this evening and tried to make a mess in the toy room with it.
He has had numerous pee accidents over the past little while as well, so I've taken to reminding him and taking him to the bathroom more frequently even when he says he doesn't have to go because inevitably he does have to after all. And I'm not talking every hour or so, he'll hold it 4-5 hours while drinking 2 cups of water in between.

So...how should I handle this? Cleaning up his messes is literally making me throw up, it's just so disgusting never mind all the extra laundry that I can't seem to get ahead of.

Any advice? Please?

He just smeared poop all over his room AGAIN when I thought he was quietly napping and I am at a total loss as to what to do. None of my friends have any advice as they have never dealt with this either.

Unfortunately, I have no advice for you, but wanted to say that we are dealing with similar issues. My 3.5 year old has phases where she has many accidents, and other times, no accidents. This morning, she took off her nightime pull-up as asked, only to poop in her pajama pants, on the stairs, and the bathroom floor ten minutes later.

She does not have accidents at school, and while she does get lost in herself, shes also refuse to go to the potty, then pee herself not a few moments later.
