Homeschool Skedtrack can be used to create a transcript. I haven't used the transcript feature yet, so I can't offer a review of how it works or how it looks when completed, though.

If the link doesn't work, create an account and then click on the "Reports" tab. You now have two new tabs showing, click on the "Transcript" tab. The quoted text above will be the first item listed.
