wakes up screaming 45 min after bedtime

My 12 month old always wakes up ~ 45 minutes after bedtime, often immediately progressing to a full scream and taking 10-15 minutes to settle back down. (She wakes frequently throughout the night, but with MUCH less fanfare.) I know that waking after 45 minutes is relatively common, but how common is the accompanying screaming? I have some theories, but can't seem to fix it...

1. Waking up in a different place (after being rocked to sleep)
2. Bad dream (doesn't seem like a true night terror though)
3. Reflux (she takes prevacid)

Anyone else? It is extremely nerve-racking and I wish that I knew how to fix it!

I'm guessing reflux - time to have the dosage compared to any recent weight gain.

This became the norm when DS was that age. He simply grew out of it around 15 months. Now, at 21 months, he will still do it once in a great while. Funny that 45 minutes was our mark, too. I could time it to the minute.

Thanks so much for your thoughts. Now I'm guessing she might be getting scared.. so last night I went in around the 45 minute mark, and foolishly attempted to pick up my keys from a table, as quietly as I possibly could. A few little clicks later, and presto chango screaming baby! It took 15-20 minutes to calm the poor girl down. We've tried white noise in the past, but may need to re-investigate that option for the early part of the night before we join her in bed.
