The problem with requiring even a birth certificate is that it's easy to falsify the information. I mean, I could walk into a clinic with my daughter's birth certificate and somebody else's kid. There's no way to connect the kid with the birth certificate. Besides, my maiden name is on her birth certificate. Would I need to carry around my marriage license as well so that I could prove that it used to be my name?
You might try putting together as bill to your state legislature that would make parents/legal guardians have to sign for all medical care, make it illegal to administer any medication (with an exception for cases where the child could die before parents would be able to be notified) to any minor without written parental permission, and make it a crime to impersonate a parent/legal guardian.
You would think that something like this would already be on the books, but it must not be in a lot of states because I hear all the time about girls getting put on birth control pills (which scare me more than any vaccine) without any parental knowledge or consent.
If such a law is on the books, it's lawsuit time. You better believe that if a clinic gave my kid that vax without my or my husband's permission, I would be searching for any law that they might have broken and suing them. I would want to set a clear precident that nobody should be vaxing kids against the will of their parents. If your mom signed a form saying she was the guardian, I think that is a criminal offense.
In any case, I would not be leaving your kids alone with your mom again any time soon...
