Yellow discharge when separation begins?

I'd appreciate your help, if this is something anyone can comment on.

Ds is 9 months old, uncircumsized. Last week he had a cold, which led to diarrhea, which led to diaper rash, which led to painful urination. We took ds to the er since he was screaming everytime he peed and wasn't peeing as often, and his penis looked swollen. Er doc prescribed a topical antifungal 2x a day, which seemed to solve the problem within 12 hours. (we went to the er 6 days ago). Ds is urinating fine, back to his usual heavy wetter status. Tonight I noticed a noticeable chunk of pus-looking stuff. Yellowish. Not smelly (yes, I tried to smell it). His penis seems fine, it's not bothering him. We can apply the cream without complaint.

My first thought was uti, especially given his diaper issues over the past week. But I wonder if he's perhaps starting to separate his foreskin from his glans? We've never retracted him, nor has any dr. He does pull at it in the bath; always has, so I suppose he may have partially retracted it himself, although if he did, it didn't bother him.

I was reading some clinical guide from Australia that said sometimes when the foreskin starts to separate, occasionally there is a discharge that is mistaken for pus.

I don't want to rush him off to an urgent care visit tomorrow to see his non-regular doctor for something that is completely normal. He's had enough dr. visits during the past week. On the other hand, I don't want to let a uti go and develop into a kidney infection.

Any advice? Is it terrible to let this go until Monday?

It's probably smegma. Smegma is just normal genital discharge (yes, females have it too.) It is very common for some to come out at separation. Your DS is a bit young to start separation, but it's not unheard if in boys his age. Possibly the the yeast infection caused it to start early.

The ER Dr deserves a big thumbs up for giving you yeast cream. All to often Dr do things like insisting on retracting, jump straight to antibiotics (which will make yeast infection worse,) or even insist that the boy needs to be circ'd. Your very lucky to have gotten a foreskin knowledgeable Dr.

Yellow discharge is not a sign of UTI, so don't be worrying about that.

Here is a list of symptoms of UTIs in young children:

Symptoms of a urinary tract infection depend on the child's age.

Older children — Children older than two years often have:

Pain or burning when urinating
Frequent need to urinate
Discomfort in the lower abdomen or sides of the back (figure 2)
Fever (greater than 100.4ÂşF or 38ÂşC) (see "Patient information: Fever in children")

Younger children — Symptoms in children younger than two years may include one or more of the following:

Fever, which may be the only symptoms
Foul-smelling urine
Vomiting or diarrhea
Irritability or fussiness
Poor feeding, poor weight gain

If there has been an infection of some kind underneath, you will probably see some kind of discharge, because the inflammation will cause more of the surface cells to slough. As long as he's not having other symptoms or worsening symptoms like fever, or swelling, or pain, I'd keep an eye on it, and apply supportive therapies, like warm tub soaks several times a day (can use baking soda in the water to neutralize ammonia in the urine), and applying liquid acidophlius 4-5 times a day to make sure the healthy bacterial balance gets reestablished.

Sounds like he had a bit of a separation episode. This is normal and the discharge is normal smegma. It is fantastic that you tried yeast treatment first. I have noticed that this is a common factor that can start the separation process in motion. The yeast often causes a bit of swelling that can spur on the separation process.
