Need this letter and I can't find it.

Anyone has the link with the letter that I can give Matthew's Ped if he tells me to sign a vaccine refusal form? I can't find it online. It was the one that listed all the risks of vaxing. I am getting ready to have some fun on Feb. 12 at his check up

all you really have to do is cross out the bits you don't like on their form and sign it.

Did I sign myself up for CPS or something by signing papers? At my ped's they had me sign a paper saying he told us about vax (he didn't at all, but I wanted to leave so badly I would have signed my car away and walked home). I thought it was something to go on file so they'd stop asking about vax's but at the last visit a week or so ago they brought vax's up again. Should I be concerned or is there something i should do about that office or the next one?

read what you signed carefully. Many of the "bad mommy" forms have a clause in there that makes it seem like you agree that not vaccinating your child is putting their life at risk. This can be used against you with CPS theoretically. Most practices have you sign form to cover their butt. In case your child comes down with a VPD they can say, can't blame us, we told you to vax!

THIS - our drs had a form that had something that basically said that but when i told the NP we were seeing that day i was not comfortable with that she said cross it off, which i did (the rest of it i was okay with).
