14 weeks old development

My 14 week old is not holding his head up - he mainly keeps his chin on his chest. I give him tummy time and he can and does hold up his head high for a few seconds. His hand are very often clinched to a fist - sometimes the thumb is out and sometimes in. He smiles briefly at some people but usually he doesn;t when we are out and about.
When I have one on one playtime with him he is totally different: he makes noises, smiles, is alert, follows me with his eyes, etc.
Sometimes he bends/ jerks his body inward, so far I thought it comes from the teething as he does it usually with a fist in his mouth, especially when he cries out of teething discomfort. I can see his teeth coming in since about three weeks. He does it sometimes when he sleeps as well.

Until now I felt all was fine but a couple of friends who are mothers of many raised concern because he does not lift his head and has his hand together. They say he is also small. But he was small at birth, and he grew 4 inches since then. He did not accumulate more body fat since his birth, but has some fat at his thighs and chin, little at his arms.

Should I worry about his development? See a pediatrician or wait?

I would see a pediatrician just for peace of mind. If there is a problem, early attention is the best course of action.

My dd is 15 weeks today. She still has her hands in a fist sometimes, not all the time though. And when you mentioned your little one bending at the waist it made me think of my dd too, she does this, with her hand in her mouth, when she's hungry. It's just one of her little quirks, I think. I agree with a pp, though, if you have any concerns then see your ped.
