exemption letter for OR?

DS (2.5yrs) will start preschool program next month and I need to file his paperwork this week.
I will need to include exemption letter in place of vaccination record and since Oregon has only a religious exemption I want to make sure I word it correctly.
I do not expect any issues from this particular school, but want to avoid anything in an "official" file that could come back to haunt me later.

any guidance is appreciated.

See if the school has one. When I enrolled my (older) kids in school I filled out what vaxes they had received and then signed the religious exemption form that was a a part of the enrollment paperwork.

I didn't really want to give them the previous vax info, but they can pull it up online so I figured I'd be honest. We stopped vaxing when our youngest was born.

It was preprinted and just said we are avoiding vaccinations for religious reasons and had the different vaccines listed. I just checked them all and signed. It wasn't a big deal at all, even though I really stressed over it.
