Houston: VBA2C with Dr. Del Castillo

I just made my first appointment for my VBA2C with Dr. Hector Del Castillo. I was very surprised that he does a routine ultrasound on the first appointment. I have *never* had an ultrasound this early in pregnancy. I'm not opposed to it, per se, but what's the purpose? I am certain about my dates.

Former patients of Dr. Del Castillo, especially VBACers -- how many ultrasounds does he do in a pregnancy? Anybody refuse or ask to minimize ultrasounds?

Also, has anyone asked to decline other routine things? He wants to do a pelvic check on the first visit. Can I refuse that? Other than the pap smear, what could he possibly need to do a pelvic check for?

What about monitoring? Does he insist on continuous monitoring during labor? I have never managed to be really mobile during labor, and I absolutely have my heart set on it. Continuous monitoring would hinder my movement.

did you ask him if he would be ok with what you are requesting? That would be a start I think.

Yes, I was a patient of Dr del Castillo! I didn't answer earlier because I really don't have a lot of answers for you. But I had a successful VBAC in October

I was not a patient of his during my early pg - I switched providers at 22weeks - so I don't know what he routinely does during early pg. But both of my old OBs did early u/s too, so that sounds normal to me.... plus I had IUIs to get pg, so I'd had, oh, 5 u/s before I ever went to an OB at 9weeks.

I'm pretty sure that the pelvic exam is a pap smear. I had one at the start of both my pg's, but not when I switched to Dr DC.

As for how many u/s... well, again I don't think I have a good answer for you. I didn't see him until 22 weeks and by that point I'd had a ton of u/s already - from the infertility docs, and then I lost a twin and had extra u/s to check on my little survivor which I could not have done without and maintained my sanity. And then I got GD so Dr DC wanted to do extra u/s to keep a closer eye on size. And at 37 or 38 weeks I started having biophysical profiles done weekly, so that's a lot of u/s. But I never had an issue with u/s so I don't know how many of those I could have skipped...

He seems like a pretty reasonable guy and is willing to discuss most things, so I imagine if you feel strongly about avoiding/minimizing certain things that he would go for it.

OK, here's the embarrassing part of my answer. I never discussed monitoring with him. I had no birth plan. I had everything so planned out with my first, that my only plan was to make it a better birth and I really buried my head in the sand a bit on this topic.

When I got to the hospital, Dr DC was on vacation (I knew this ahead of time, he told me his vacation dates at my very first appt with him, his vacation started the day after my due date.). I also had GD. I was dealing with one of his backups, who says he does vbacs but didn't want to do one on me since I had GD, was overdue, and frankly I think just because it was 11pm on a Sunday night and he wanted to go home to bed. (I think mediocre backups are Dr DC's biggest drawback as far as vbacs go). And so I didn't even pushback against the monitoring. In the early stages I paced around the bed, sat on the birthing ball, and make frequent long trips to the bathroom, but I was mostly on the monitor. At 6am I asked for an epidural, Dr DC got there at 7am as they were placing the epi. So I don't know what he would have said about the monitoring.

I hope some of that helps though I don't have many answers that you're looking for. I did like him, I'm glad I chose him. Oh, I also want to add that there were several points through my day of laboring that if he had suggested a c/s I would have gone with it. He never did. He had more patience than I did, and I'm very glad that he did.

ETA: Oh, the other thing I liked about him. The first time I met him I was fully clothed and in his office, not an exam room, so I felt able to talk/ask questions as much as I needed before I went into an exam room. I hate meeting a new doctor sitting there in a paper gown.

So I have never been his patient, but have worked with him in the past. Here is what I know (from him & other docs I have worked with).

Most like to do a pelvic & pap during the 1st visit to make sure there are no surprises (cysts, masses, stds, infections....). The only way I know for sure to get out of it is if you have been their patient for a while.

The early ultrasound is for dating purposes. Unfortunately, there are many women in houston who have no idea what their dates really are (I run clinics including prenatal), so a lot of docs have resorted to early ultrasounds for everyone so EDC is correct for the future (esp if you want genetic testing). Unless there is a problem, Dr. D-C will probably just want the anatomy scan at 20 weeks.

Since I only had go go in on high risk deliveries, all my babies/mommies were monitored, but a lot of mommies at hermann do the intermittent monitoring thing.

Go in and talk to him about it on your first visit, ask him to explain risks & rationales for each test, since you want to be as non-invasional as possible.

My first appointment with him is on Wednesday. I was just hoping I could get an idea of what I am walking into before I go. I am just so nervous because I haven't had an OB since my second baby (and this is my sixth).

Is his clinic at the Museum District? I read at another forum that he delivers at Memorial Hermann at the Medical Center. Do you know if he also delivers at another hospital? Is that MHMC the one you used? If so, how was it? Thanks. :)

HealthGrades says Del Castillo is at Park Plaza, though I know my OB who was at Park Plaza also had privileges at Hermann Med Center, where my first was born. If you give his office a call, I'm sure they can tell you. Or does he have a website?

Dr. Del Castillo delivers at Memorial Hermann.  He is one of the top OBs period, but one of the best for VBACs.

I am a patient of Dr. Del's and I had a wonderful experience with him for my daughter's birth in Nov, 2011.  I too switched to him late in my pregnancy (30 weeks), and he was very open to my birth plan. He allowed me to go two weeks over my due date (and probably would have allowed more), and then we induced. He did a very minimal induction, and I had no pain meds. It was a longer labor at Memorial Hermann - 32 hours - but I was still able to deliver vaginally the way I wanted to.  All of the nurses there were great, and they told me I was so lucky to be with Dr. Del because any other doctor there would have already had me in OR for a csec.

 

I highly recommend him!
