Baby A HAS to be head down - what can I do?

I'm almost 13 weeks pregnant with twins. Because of where I live (Louisiana) I can't see a midwife for this pregnancy. I've found an OB who is pretty hands-off and natural birth friendly, and I'm comfortable that he's my best option for this pregnancy. While there are a few hospital policies that I can't get around (I have to deliver in the OR, have to at least have a hep-lock, etc), for the most part he has been very supportive of the way I want this birth to go.

The only major issue is that in order to attempt a vaginal delivery, Baby A has to be head-down. If I deliver Baby A vaginally and Baby B is breech, we will either attempt to turn him/her or have a breech delivery.

I know that it's still early in my pregnancy, but I want to know what I can do to give myself the best chances of having Baby A present vertex. Any information you can give me would be greatly appreciated.

I wouldn't even worry about this until your third trimester or so. Perhaps a little earlier because things get crowded earlier with twins.

My twins were A, head down/B, breech. We did a breech extraction with ds2 (the OB reached up into my uterus, located his feet, and guided/pulled him out...yowza!)

Honestly, I think you have less control with positioning when you have twins. It's not just up to what you do or what positions you encourage...it also has to do with the other twin and what they're doing to make baby A uncomfortable or comfortable in that position.

I would think all the spinning babies stuff applies...do your pelvis tilts, spend time leaning on a birth ball or going hands-and-knees, and be aware of your posture (no reclining positions).

Good luck! And remember, as the twins get crowded, they'll do a lot of flipping. Don't let that worry you! Things settled down quite a bit once I was close to term.
