Looking for letter for dr. reverse aap stance

I am looking for a letter I had given to my doctor back in early 2008? or late 2007 .

It basically was a modified letter from what the doctors wanted me to sign.... any one know what I am talking about,

if you search on google for modified app refusal form something will come up.....but in my experience doctors will not sign any modified paperwork, but ask for you to type up your beliefs that they will include with their statement.
