Another eczema question

DS2 (who turned 2 last December) has had eczema since infancy (~2-3 mos old) which comes and goes, never completely goes away but runs from mild to moderate, not typically severe...has always seemed to get worse in the warmer months and better in the colder months, so I've thought it was environmental in nature.

His flare-ups are typically all over his body (except face), but worst on his ankles and wrists b/c that's where he scratches himself. Recently had his worst flare-up ever, after being almost entirely free of it earlier in the winter. Over the past several weeks it's degenerated to the point where he developed a secondary staph infection (his ankles and wrists are a scabby, weepy, painful mess ) We took him to urgent care yesterday and they prescribed abx for the infection. We're supposed to air out the wounds when he's at home (which seems impossible b/c when they're not covered he scratches them again...) and cover them when he's at daycare.

The puzzling thing is that a couple months ago we took him to an allergist, who did a blood test which came out completely negative for EVERYTHING.

any thoughts on why this might be? And what we can do about it? It seems like there are so many possible triggers, and possible ways to address it, that I am completely overwhelmed.

We've never done an elimination diet...even the thought of that overwhelms me, I work fulltime and I can't even begin to imagine planning a whole new set of meals (when i barely have time/energy to cook to begin with).

Most creams/lotions do not work well for him. I've been using 100% shea butter lately, which is a great moisturizer but seems to have zero impact on the itching.

I need help! I hate to see my poor DS2 suffering like this.

Eczema can be from environmental allergies, food allergies, or food intolerances. Not sure what they tested for, but depending when he was tested (and whether it was RAST, or scratch testing) it's not very reliable under 2 years of age. And if it's an intolerance, it won't show up on those tests anyway.

You could look at the top 8 allergens, and the top 4 intolerances (dairy, gluten, soy, corn). If it is flaring up now after all winter being good, then it could be a food that just started appearing in the grocery stores (what's in season where you are?) or an environmental allergy. Has anything else changed?

My pediatrician told me last week that there are a very high number of false negative allergy tests...
