Intact Toddler - questions about redness/irritation

Hi all,
I have a couple of questions/concerns I hope BTDT moms can advise.

My intact 2.75 yo is peeing very frequently recently & has redness on the tip of his foreskin so I'm a little concerned.

Some details: he's still ballooning while peeing, which I am fairly certain is normal.
He's recently become more "aware" of his penis, and tends to play with/pull it more often.
While peeing recently, he is pointing it down, which I've been advising him to do so not to spray while sitting (he's been potty trained for about 9 months), but he's also pulling on the foreskin while/immediately after peeing - like more pulling & pressing - like its irritating to him, rather than just pointing it down.
The end of his foreskin for the last few days, I've noticed, is very red. My assumption is this is due to the amount of pulling he's been doing. He doesn't say that it hurts, but I'm getting a bit concerned because of a couple of things:
1) He has been having a lot of accidents in the last couple of days - obviously, I understand he's still 2, but its like 5 pairs of pants per day, and he's been out of diapers for a good long while now.... so it makes me feel like something is up.
2) When he pees, or directly afterwards, he makes a grimace/grumpy face. I've asked if it hurts, and he says no, though he has said, I don't feel good when I ask why is he making a grumpy face.
3) When I ask him if he needs to pee, he immediately says no, then proceeds to wet himself - but in small increments - like he's not fully emptying his bladder. He's basically urinating every half hour.

My concern is a bladder infection, yet, he has no signs at this point other than frequent urination.

My questions are: is the ballooning still normal? Is redness on the tip normal? Is there anything I should be aware of / to check for beneath the foreskin that would indicate an infection? At bath time, he washes outside with California Baby & rinse/swish in bathwater - though I do have him pull his forskin back a bit to rinse - he's not retracted so it doesn't expose anything internal point, just want him to get into the habit of swishing/rinsing well now while he's a toddler, so that when he's older, and I'm no longer helping him with bathtime, he washes completely. Could potentially getting california babywash under the foreskin cause irritation?

Is there anything topically that would be safe to put on his penis if it looks like its just a scratch or scrape that's infected? Does it sound like a UTI or is there other cause for frequent urination & irritation?

I can certainly take him for a UTI test but don't want to have the doc retract at all, nor use a catheter. I'll be sure to ask to have him bagged, or have him sit on a "top hat" since he's potty trained.

Any thoughts or advice is welcome. Thanks.

If you suspect yeast then you can use OTC monistate. If you try that and no improvement then you can try OTC bacatracin. It is like neosporine but is safer to use on the genitals.

I had a similar problem with my potty training 2 year old. I think it's the start of seperation combined with lots of extra attention. I gave him a couple baths with just baking soda, no soap and it seemed to have cleared up.

If you really wanted to you could get those home UTI test strips and have him pee in a cup just to see if it deserves a trip to the dr.

When DS was that age (he was also an an early potty learner) e liked to play with it right after peeing too. I suspect it was b/c the sphincter at the end of the foreskin was relaxed at that point and it made it easier.

DS also would go through periods with very frequent accidents. There would be times early on when the novelty of the potty had just worn off and it didn't seem worth the trip. Any time he had a growth spurt it also caused frequent accidents, it just seemed like he lost touch with his bladder signals at it's new address and needed to get reacquainted.

No, there is nothing to see in there and you could hurt him trying to.

Yes, detergents under the foreskin and overly vigorous washing will often cause irritation. The intact penis should be treated in a similar manner to the intact vagina.

Though you can use Bacatricin (never use neosporin) I would avoid it with out a confirmed bacterial infection since it can throw off the flora and cause a yeast infection. Treating for yeast with monistat is safer, but I don't think necessary in this case.

It doesn't sound much like a UTI, but these things will vary from person to person. There are a few possible causes for frequent urination, a serious and common one is diabetes, so if it continues you should get his blood sugar tested. Irritation happens for so many reasons I could spend hour sitting here listing them, but since you mentioned using califonia baby on his penis and his playing with it those would be my bets.

They don't need to see him to do a urinalysis, they just need a clean sample to take to the lab. You can just pick up a specimen cup and have him pee in it then drop it off. This way you avoid the Dr trying to play with the foreskin.

Thank you everyone for your replies.

Just an update - The frequent peeing seems to have resolved itself, and the redness seems to come and go. In paying closer attention I've noticed that's definitely associated with the more he handles it; guess that's just the way it will be for the next, forever?

I'm going to cut back on the amt of babywash. We do only use California Baby which doesn't have detergents in it. Its the most gentle baby wash I've found (use it myself). We'll just be a bit more conservative.

I've also upped the probiotics.

If nothing else, this issue allowed me to do lots more reading & researching on being intact. So we're all more educated. Which is always good! Thanks again ladies.

Next up: Discovering standing up to pee. Oh boy.

It does have detergents. They are just much more gentle than the detergents typically found in skin care products.

Ballooning is 100% normal and will continue until the opening widens throughout childhood/adolescents. My son is 26 months and has been ballooning since he was 10 months.

Yes, this is a normal symptom of separation and/or irritation. It usally doesn't require additional treatment. A soak in a bath with 1/4 cup of baking soda can help soothe any irritation.

No. never try to retract him and at this point you shouldn't even be asking him to try to retract. This is unnecessary and can damage the delicate opening sphincter. Infection will get very bad in a matter of hours the entire penis and groin area will be very swollen (like 5 times normal size) and he will get a fever.

You should not be applying any soap to his genitals, this can cause an imbalance of the natural flora. Just a swish in the water, you don't need to even touch his penis. Never let him soak in soapy water. Wash him, rinse him and refill the tub with clear water for him to play.

Try a little diaper ointment first. I use this whenever my son's tip gets irritated. See if that works first.

I wouldn't jump to this conclusion yet. Try giving him lots of water and a little diluted cranberry juice.
