I would be worried about the slow weight gain together with the constipation and anemia. FWIW, she weighs more than DD2 did at 18mos! She's 21mos now, just realized she's wearing size 9-12mos pjs--too short in the arms and legs but loose on the body, butt and legs. She's just slim, some people are, but she has been gaining steadily (if slowly, was 8lbs at birth then dropped right off and now hovers on the 3-5th centiles)
I do feel she needs all the calories she can get, so here's what I do:
don't let her snack on crackers etc--if she needs a snack it's yogurt, fruit, toast, more like a mini meal
make sure her meals are as healthy and high calorie as possible, so that if she only eats a bit at least she's getting good stuff. We eat lots of fish and chicken, homemade mac n cheese, cereal with whole milk, whole milk yogurt, lots and lots of fresh fruit
ALWAYS have dessert--this is like the chocolate ice cream thing. I count a healthy dessert like good ice cream, fruit crumble with cream, puddings or custards as part of the meal.
ETA: I try very hard to be relaxed about what DD2 doesn't eat. Now that she's consistently eating well, one skipped meal or an 'off' day doesn't bother me. I plunk down the food and let her decide how much, if any, she eats. Very tough with a skinny baby! DD1 is a fab eater and has always been a good gainer and grower so I've had practice not worrying, but still!
