exemption form?

ok so my DD will be 6 this year and we are homeschooling her,and i was wondering about the vax exemp form we did get one from our county health dept when she was 4 to put her in a monterssori school which lasted a week and we took her out, so that school has the original form, do I need to go to health dept to get another or can i ask them for that form back, dont know if they even have it , it has been two years since then, please advice.

If you're homeschooling, in most states you don't need an exemption at all. They are only for school attendance.

Yeah, you shouldn't need one for homeschooling. But if you do need one, I'm pretty sure you can get one from the county health department.

In Texas, signed forms expire after two years.
