HELP Please: State coming to home daycare - Vax Exemption Letter

Hello,

I have my 20 month old son in a wonderful home daycare and I was informed that the state is coming this month to do an inspection and I need to present a MA Immunization exemption letter. I have some on file (not filled out) and also read some others that are out there but wanted to know what you use b/c some sites say to use your own language vs. the ones from the internet. Is there any truth in this? So I am just looking for a simple religious exempt letter but felt uncomfortable with the one provided below again b/c of the above comment.

I haven't heard any complaints of daycare health department checks resulting in problems for the parents. These seem to be paperwork checks; just to make sure the dc has records/exemptions on file.
