UP record-keeping

What records have any UP-ers kept? I haven't been in this forum for a long time, since my last UC, but I vaguely recall that someone (or several someones) had a full binder of forms/tests/etc. that in the case of, er... legal issues... that they hoped would back them up. Like, "here are my records, I got some prenatal care, obviously, I'm not irresponsible." Plus I imagine, even if it wasn't legally workable, if there WAS an issue that cropped up I imagine records would be helpful to a midwife or OB to look through. I do tend to have some issues with pregnancy (quite a few) but nothing that in my experience that the midwives have managed any better than I myself have.

I've used a prenatal chart to log my weight, blood pressure, pulse, fundal height, urinalysis, and fetal heart tones. Really, it is for "back-up" in case anyone tries to create problems for me, and a little bit for my own satisfaction, although I would not feel the need to actually record everything.

I PM-d you, and you might already have answered it in your email, but just in case - how often do you keep records? Weekly? Monthly? Some other interval?

Sorry, just now reading this. I emailed you the chart as an attachment

I did my own "prenatal" every 4 weeks until I hit 32 weeks, then every 2 weeks until I hit 36 weeks, then weekly until birth.

I just did my weight weekly, and at least once a month I did FHT, BP, pulse, etc. I also check myself (because I want to and I find it fascinating, not because I'm gauging progress ) and I'd keep track of that too.

Ditto to this. I have done mine monthly though once I get to 36 weeks (1.5 more weeks! ) I will probably do it weekly.

I've created a chart in Excel to fill out and it graphs my weight and fundal height. I also slightly altered a medical history form in Word to go with it incase we end up transfering to the hospital.
