Right now, it's only about once a month. At the moment though, the only fish we have in the house is trout and I'm kinda bored with it. When we do buy other kinds, it's more often.
I almost always bake fish and have rice and some kind of veggie on the side. Except shrimp. The only way I ever cook shrimp is in paella.

For white fish, I just rub with herbs and pan or oven fry, serve with veggies and potatoes (or whatever sides you like).
Salmon is really yummy in the oven, served over salad, blackened. I've also made salmon burgers, and I have a recipe for salmon rosti that I really want to try.
I have a casserole-ish type bake that is carrots and potatoes with white fish. Season with dill, garlic, paprika. Top with parm.
I can give you recipes if you like, but I mostly make stuff up.

Oh, we're talking about shrimp too? I do shrimp in a pasta dish with tons and tons of veggies and lemon butter and herbs. Shrimp are also good on salad. I make a few creole dishes that also have shrimp.
We do fish about 2-3x/month. I would do it more if it was cheaper to get good fish. Cheap fish just isn't worth it. I used to live in Seattle and we would eat fish there at least once a week. Now that we are landlocked, it's more of a special occasion thing.
The best way to do it is to look for whatever's on sale and plan around that if you want to eat more fish.

We eat fish 1-3 times per week. Fish is expensive so we mostly used canned salmon for salmon salad sandwiches as me and dd love sandwiches, but I don't usually eat luncheon meat. I also sometimes make salmon patties/burgers with canned salmon.
Maybe 2 times a month we have shrimp, usually in a stirfry or pasta dish.
We rarely get fresh fish, though I'd like to.

We eat fish, probably twice a week. My kids eat salmon salad in a sandwich at least once a week. I don't actually LIKE salmon, so I usually make mine chicken salad, but I want them to get the good fats. We also eat fish as a main dish at least once a week; sometimes it's salmon, and I eat just a small amount. I like mild white fishes, like tilapia, but it's hard to find that not from China. I like flounder, or cod, wild-caught, when I can get it. I also like halibut. DH buys them catfish or trout and cooks it himself; again I'm picky about those. I really like swordfish, but I won't eat it because of the mercury. I'm thinking though that now that I'm sure I won't be having more kids, I might indulge, just me by myself, once in a while.
I don't buy shellfish very often. I don't really like it too much. It comes from working in a fish market as a teenager, I think. I do use canned clams to make chowder, probably once or twice a month, and my mom makes the kids shrimp sometimes.
Anyway, we eat our fish mostly baked or gently broiled in butter, or sometimes with garlic and butter, or topped with raw tomato in season, and often with lemon. We like it with rice or quinoa on the side, and with green veggies-- especially kale cooked with garlic, or buttered and peppered sugar snap peas when they're in season.
We do have a lot of concern about overfishing, and about the environmental impacts of fish farms, and about the contamination of the fish, but I believe that the omega-3s we can absorb so easily from fish are essential for human health. It's quite a dilemma, because if everybody on earth ate the amount of fish that's really healthy for them, there wouldn't be enough fish to go around, and not a single fish left.

I'll qualify this by saying that I've never been a fish person, but I've recently been convinced about the importance of eating fish frequently. Although I never used to like fish, the more I eat it, the more I like it.
We eat fish 1-2 times/week. Most of the time, it's just a marinated fish filet, served with steamed or sauted veggies. I usually buy wild caught frozen fish in large packages. I occasionally buy fish that has already been marinated, but it's hard for me to find pre-marinated fish that doesn't contain hydrogenated oils. So usually I just buy the plain filets.
I grew up eating salmon patties and tuna casserole, and can't stand either of these dishes.

We eat fish about twice a week. Our family does not eat chicken or red meat so fish is kind of an important part of our diets. We eat baked salmon (yummy!) once a week and either baked or fried cod once a week. I like getting fish tacos when I'm out, I don't know why we don't make them at home.

We eat fish once or twice a week (usually salmon, cod, halibut, sole, trout, and tinned salmon in sandwiches) and shrimp maybe once a week.
I grew up in a fishing area of the UK and now live in Seattle where we're spoiled for choice. I know overfishing is an issue but I try and stick to more sustainable fish (and avoid Chinese stuff).
Ways I cook it: usually grilled (broiled?) with olive oil, lemon and salt, sometimes fish pie (poached fish with mashed potato on top), fish/shrimp tacos, shrimp and garlic pasta, baked in the oven with herbs, then tonight I did tuna meatballs (Jamie Oliver recipe).

often, it's whatever sustainable, kosher, inexpensive fish our fish monger has, so we've tried a lot of new things. Sometimes as cerviche, sometimes we'll buy (from a different fish monger) sashimi grade tuna trim(8-9 dollars a lb instead of 21. we buy half a pound. great deal, as it tastes like a LOT of tuna for the two of us.) and make poke/tuna tartar. We most often pan fry. Recently, DP made some breaded pan fryed cod that was scrumptious.
I LOVE salmon grilled on a plank, which we eat a LOT at my dad's. However wild salmon is too expensive for us, and we don't have a grill

we eat fish 3-5 times a week, I included next day leftovers in that number too. Mostly we eat salmon as we are 5 minutes from the river, dh and dd2 are halibut and crab fishing right now. (ocean is an hour away) so fish is very local for us. mostly the fish is grilled when weather permits otherwise baked and then made into fishcakes, chowder, or sandwiches.

I sometimes do a tuna melt... Make a pizza dough crust and top with a mixture of: canned tuna, miracle whip/mayo, onion, granny smith apple and of course some marble cheese.
But, I love baked salmon. A lot of times we just put herbs on top but, we've done maple syrup and bbq sauce, which is awesome! (I have a wicked sweet tooth)
I wish we could eat fish more often--I mean, good fresh fish--but it's expensive and we aren't in a good location for it.

We eat fish once a week and then other seafood (shrimp, scallops, mussels, clams, etc.) another night, so twice.
I like white fish pan fried or baked...poached in milk is yummy too. We also love to do homemade beer battered fish and chips. We do salmon burgers, tuna or salmon and pasta bakes (a la Dr. Sears). I make salade nicoisse pretty frequently in the summer with tuna in it. Also in warm weather we make fish on the grill...steaks like salmon, tuna, swordfish bluefish, striper. Shark-kabobs are a treat!
I make stews, too. Boullaibaisse, chowder and heartier dishes like paella. Shrimp and baby spinach risotto has been a recent favourite.
Mussels in simple wine broth with fries on the side...a dish I grew up with in Belgium...yum! Scallops and green and yellow squash sauteed in butter over linguini.
We love seafood. Getting great ideas from this thread...great question.
ETA We also love asian-inspired fish in parchment paper. Dash of soy sauce, fish sauce, grated fresh ginger, sesame oil (and seeds) topped with veggies like sliced carrots, bok choi, pea pods, lime wedges. Fold up the paper. pop it in th oven and it steams itself. Lovely.
Oh, and we love making sushi. If you can stand the mess, it's lots of fun to do with kids!

We eat fish once a week. DH was a vegetarian for 20 years, and just added fish back into his diet about 3 months ago, so for now I am focusing on the lighter white fishes like tilapia, cod,and halibut.
For the most part, I spray a griddle pan with cooking spray, and shake cajun seasoning on the fillets. Then I pan fry them, brushing on a little extra canola oil on each side to give them a bit of crispness. It cuts down on some of the fat of "frying" but my DH and child both like the crispiness.
I also put filets in a dish in the oven, covered with diced tomatoes, garlic, herbes de provence, and white wine. That is really great, and easy to make.

We have salmon once a week. I only have one recipe: I blend tamari soy sauce with fresh ginger and garlic, add a little maple syrup pour it over the fish and bake it in the oven.
We would like to start having salmon twice a week but I need a new recipe so we don't get tired of this one!
Once in a blue moon we have the enviromentally safe tuna but we are worried about mercury even in that so not too often.
Once in a while wild haddock with tahina sauce.

We love fish but don't eat it as often as we'd like.
We eat salmon once a week usually. I have a few great recipes that I rotate- salmon baked with a fresh ginger, cumin, coriander + oil rub served with a yummy mango+chili pepper + cilantro + lemon juice salsa...so good. Also just poach salmon and use the poaching liquid to make a quick creamy sauce with flour, dijon mustard, parmesan, salt and pepper. Those are my favourites.
For white fish my favourite recipe is just baked with potatoes, tomatoes, peppers and a charmoulah sauce- look it up on epicurious, it is deeeelicious.
For fresh caught, whole fish we just stuff with herbs and lemon, add salt and pepper and throw it on the grill.
I also like to make a simple pasta sauce with tinned tuna, kalamata olives, fresh tomatoes, capers, and garlic + olive oil. Sublime!
DH makes a great homemade mac and cheese + tuna with whole grain pasta, homemade cheddar sauce and just chucks a can of tuna in.
