UC in Ohio - BC question

Anyone here UC in Ohio? I have a question about the papers for the birth certificate. Specifically the Pregnancy Verification and Infant Verification. Who did you get to sign it?

I didn't UC in Ohio, but I home birth in Ohio and we had my friend sign the papers as a witness since she was present for birth. We had a little hassle with the paperwork, but it wasn't near as bad as I thought it would be to be honest.

The midwives helped us fill out some of it, but the rest of the paperwork we did ourselves.

All though I am a little foggy on things, but I could go back and pull the paperwork out of the filing cabinet and check any further questions if you would like!

Who did you have sign your Pregnancy Verification and Infant Verification for Out of Institution birth?
