Birth Certificate in Kentucky

Any new info for obtaining a Birth certificate in Kentucky? I would like to get a packet before the baby is born.

We had to take all the "proof" to the registrars office and fill out their form. They would not mail out the form, you had to do it in person. But it was really easy.

My UC baby was born in Glasgow, KY (not sure which county that is.) The health department had NO idea what we needed to do as they had never had a UC before. We all just went down there and filled out the paperwork for a birth certificate. They never asked for proof of pregnancy (even though we had a confirmation of pregnancy and an u/s photo just in case.) It was much easier than we thought it was going to be. Putting the dad on the BC was the most complicated part because we weren't married and had to fill out a paternity affidavidt, show them our ids and get everything notarized.
