Expanding a pattern

I'm trying to make a diaper cover using the Very Baby Diaper pattern. They talk about adding 1/4" - 1/2" all the way around. Does that include the fold? Or just the outside edge?

I don't want to giant size my diaper cover because I misunderstood... or make it too small/narrow.

Thank you! I thought that might be the case, but it's one of those things I really wanted to hear from someone else before I started hacking up my fabric.
