Help with New York State religious exemption

Hello!

I've had a religious exemption for my son in the state of Florida for 3 years. I had no problem obtaining my exemption in FL, and they didn't even so much as ask me one question-- show up at the health dept, bring in my papers, signed, stamped, and on my way.

I'm moving to NYC this summer and my son will be starting kindergarten in the fall. I'd love to any information or guidance on how to fill out the forms correctly, to avoid any trouble (if possible!)

I wouldn't take any chances. Id contact Gary Krasner for advice. Another mom from NYC just got denied. I'd be pre-emptive and contact Gary from the get go.
