You will have to get one from the health department. The daycare doesn't have to accept an exemption unless they receive funding from the gov. Most centers do, but private home daycares might not. Anyways here is the wording:

If they are licensed they would have to accept exemptions because they are subject to the vaccine law just as schools are. If they are more of a babysitter/casual operation then I guess they can do what they like.
