Lost my list of questions to ask midwife! Nooo!

I'm meeting with a midwife tomorrow and I had a very long, very detailed list of questions to ask her. It was a masterpiece; I had been working on it for a month. Unfortunately, my computer crashed yesterday. I managed to save some files, but all my Word documents were erased! I'm trying to rebuild my question list in the time I have left, but I can't remember everything that was on it!

I've been googling "questions to ask midwife," but I'm hoping some of you ladies could suggest some more detailed questions, the kind that are important but you might not normally think to ask. Thanks so much.

Transfer rate for first time mamas.
Policy if you go past 40 weeks.
Policy if baby is breech, transverse, you're gbs+, or develop GD.
How many clients are due each month?
What happens if you're in labor and another mama is too.
If you transfer what does she do...go with you? Labor support?
If early on a complication develops and it's known you'll be delivering at a hospital what happens? Payment? Her role?
Do you see a MD for backup? When? How often?
What tests is she okay skipping? GTT? Gbs?

What is her labor philosophy? Lots of moving around? Laying down? Following your ques?

Seems most midwife's are cool with most MDC stuff...delayed cord clamping, no vax, no eye goop, baby only being away from mama in emergency, no episitomy... I didn't focus much on asking my MWs about these things during their interview. Now, knowing my MWs over a year through two pregnancies, I know through our conversations during prenatals that they'll do what I want and what's good for baby. Like eye goop...it won't happen unless I have a bad culture (I get lots of cultures due to past infection).

A question I asked that I felt was important back then but now see is trivial is if they've lost babies in labor. In most cases a baby loss has nothing to do with a midwives care.
