request an evaluation from your school. The request needs to be signed and dated, so write a real letter on real paper and mail it. The have 45 days, but I believe that only days that school is in session count. (If you are concered they with blow you off, get a signed reciept, but I never do that because our district is wonderful).

It sounds to me like he is gate keeping to keep cost down. I'd try to go around him and get referral for her insurance to pay for a private evaluation in addition to the one through the school because they tend to be more thorough. (I perfer to do both the public and the private option).
The type evaluation your son most likely needs is a neuro-psych evaluation.
Some of these issues tend to get much worse during adolscence, so ignoring severe anxiety in an 8 year old is bad idea. However, some of this stuff can take a long time to sort out. For my DD's last eval, I wanted the best person in our area to do it, and it took nearly a year between being on a wait list, having multiple appointments, and then waiting for the report (my DD has PDD-NOS and homeschooled until she was 12). My advice is to start the ball rolling but don't hold your breath!
Dyslexia is a separate issue and requires separate testing. It's not something I'm up on, and I'd recommend posting a specific question about it on the special needs board.
How are your son's social skills and fine motor skills? Kids with some of the issues you listed often have trouble with those as well.
