So sorry this is long....
It all started when DD had uti symptoms one morning - stinky urine and then when tested, a positive home nitrite test - WBCs were negative. I treated her w/d-mannose for a few days and symptoms went away for a day, but then came back. So I increased the dosage (staying within normal dosage on the bottle) and her symptoms went away again, but came back a few days later. With the exception of the first two days, all symptoms are only with the first morning tinkle. This went on for almost a week. During this time she also had a tinkle accident on the family room floor (she is fully potty-learned). Thinking she would probably need an antibiotic I stopped the d-mannose on a Wednesday night and called the doctor to get her urine tested. The sample was collected and dropped off at the lab on Friday morning. I was to call Saturday to get the preliminary results, and if necessary a rx for antibiotics. That whole Thursday she was negative on the test strips and no funky smell. But Thursday night she developed a fever (101.6). Friday morning the fever was gone. Anyway, after the urine sample was collected and taken to the lab on Friday morning, she had an appointment w/the doctor – I made the appointment because of the fever (it scared me). Our regular dr was off that day so we had to see someone else. She (the dr) told me that nitrites should not be in the urine (even if the WBCs were negative) and prescribed an antibiotic for dd to start right away, with the weekend coming up she didn't want us to end up in the er if symptoms progressed. Later that night dd had two tinkle accidents within 2 hours and the following morning she woke up wet so I thought she surely had an infection and started giving her the antibiotic.
Well, to my surprise, the test came back negative. I told the nurse who called with the results that I already started the antibiotic and didn't know what to do now. I was very upset and practically in tears. She said she would have our regular doctor call me back.
He told me that sometimes you can have a little bit of bacteria in urine that has been in the bladder for a long time. And that this doesn't necessarily indicate a full blown infection. He also said, on the other side, that you can't always go by test results or a low number of colony units, you have to take into consideration additional symptoms and how the patient is feeling. He said that since I had already started the antibiotic AND since she was exhibiting signs/symptoms of infection that I should continue w/the antibiotic, but could shorten the duration from 10 to 7 days. Also, since she was high-dosed and having some side effects, that I could lower the dosage a little bit to be more appropriate for her weight– which I did. If symptoms return, then we would need to get a catheter sample and redo the urine test. UGH! FTR, He is very conservative w/antibiotics.
I keep thinking that somehow the d-mannose skewed the test results. IDK. ? But, she has had uti before and I do know the symptoms and signs to look for. This is also why I always keep test strips and d-mannose in the house.
So here I am, we just finished 7 days, but I think we should do the full ten anyway. I just do not want this to come back if it was an infection and ten days is a typical duration for this antibiotic – amoxicillin. Since she has started it there have been no accidents, no stinky tinkle, no fever, no positive test strips, no other symptoms.
So... would you stop now at 7 or continue for 10?
