MSWord has an Immunization Waiver template

While looking for a letter template, I discovered that Microsoft Word has an immunization waiver template.

I don't know how helpful it would be to anyone, but I am more enthusiastic about the fact that it could inform some parents that choice about vaxes is possible.

What version of Word and how exactly did you get to this? I have 2007, and I went to Templates, Forms, Healthcare or something like that and didn't see it.
