Mom with Boxing gloves and outline for Neuro

I am going to get a Dx by Sept when DS turns 2 if it kills me.
We go back to the Neuro today.. I am comming in what papers. Pictures and trying to get my brother to burn a DVD of movements for over the weekend

Can anyone tell me if this is to much or to little or should I be more detalied or less detailed..

Trouble Planning Movement and Gross Motor Concerns
Falling
Tripping over nothing
Bumping into things that have always be in the same place
Vision appears normal
Taking a long time to master new gross motor task
When mastered it often has to try several times to complete
Thinking to try to complete motor tasks like stairs or climbing
Waiting long times to do the next step in a motor series.
This is intermittent but does present struggle
Appears to roll ankles often or fall due to weak legs or ankles
X-Rays are all normal
Seems to have hands more in a curled ball will relax for short time but kept closed.

Lazy Movements
Crawling after falling to get to item or goal
Not holding spoon correctly
Used to have this skill mastered
Will not sit with strait legs on the floor or bent legs in a chair or car seat
Appears to have to have hips bent to relax
PT done once weekly OT didn’t qualify.

Verbal and Eating
Not talking
No words
Speech done privately 3 days a week in home.
Signs ~ More, All Done, Want and Help
All signs are done but look weak like he us unable to complete them.
Babbling
Slight increase but still less than 10 min a day.
great thought and effort almost forced to get sounds out.
no damage and structures are normal
hearing normal
Eating
Not chewing food
Will mash or let dissolve in mouth
often gags due to this method
Feeding team in September




Parent Concerns and Updates
What is the delay and why?
What else can be done?
What is a diagnosis and prognosis?
Parents have left EI due to lack of services
Paying for private services.
EI was not working with parents
Head holding and light sensitive when tired.
No video camera to document
happens about 3 times a week
Lays down hold eyes and or head around the time of nap or tired.
Holds eyes and head when changes in light happen quickly.
Vision appears normal but no vision test done with Ophthalmologist yet and nothing scheduled.
Paperwork has to be filed with Taunton Public Schools or need of service.
School asking for diagnosis or areas of need
Parents want diagnosis.

That's a pretty comprehensive list. That plus the DVD of the movements should give the neuro a better idea of what the issues are.
