a question for CNMs

I am wondering do you need a back up doctor to practice as a nurse midiwfe? If yes then could you please elaborate on how that works out.

My CNM's are a practice of four and they are in the same office as a large OB practice. They bounce things off the OB's if something is out of their scope of care. With my first I never saw any OB, with my second I risked out of the hospital they practiced at and had to deliver at a larger hospital with a high risk OB.

In differs from state to state, but CNM's cannot attend homebirths nor can registered nurses as Doula's even. It jeapordizes your license.

I do know that it varies by state and even hospital though how the back up and referral to an OB works.

In my state, a CNM can legally attend a homebirth. I think they used to need an OB "backup" but I *think* the regulations just changed on that. My midwife practices in 2-3 states, so she has an OB "backup" but according to her "does not really influence my practice".

You should check your states laws. Someone in the local tribes may be able to help.

in Az CNMs are independent advance practice nurses- they do not need a doctor back up. they do need to have a plan of how to transfer -
but there is generally about 1 CNM in the state who attends homebirths the rest of the CNMs are prohibited thru practice agreements they sign with the docs that work with them and they are hemmed in thru malpractice agreements they have as well-

there is a requirement to have a collaborative relationship but doesn't have to be in writing-
I would say call some local CNMs and ask how they do it
