I can snap a photo if you need to see it...I have no idea where to start on what could be causing it.

How is he doing?
I came here looking for advice on this same issue. My 4 yo and almost 6 yo have had a persistent rash for about 6 months. DH was using a heavy duty topical steroid (which was his for poison ivy) to keep the inflammation down and unwittingly disguised the scope of the problem.
We finally stopped the steroid and 4 yo DD2 (who has it worse) just finished a 10 day course of an oral antibiotic and a prescription antibiotic ointment. It's clearing up but nowhere near gone and still not as mild as <6 yo DD1's was. And even DD1's is still showing despite being on the same topical antibiotic.
Anyhow, I found this Dr. Sears article in an earlier post on this forum.
I think I'll try the more rigorous regimen for a couple of days and see what happens. Otherwise I'll probably ask the doctor for another course of antibiotics.
Good luck.

If the infection continues to worsen despite the above treatment, then proceed with the following steps three times a day:
Wash with warm soapy water.
Apply a hot washcloth to the area for 10 minutes (this will improve blood flow to the area and help the body fight the infection).
Apply diluted hydrogen peroxide, than wash off after two minutes.
Apply 10% diluted Betadine solution (mix 1 tablespoon of this over-the-counter brownish red antiseptic with 10 tablespoons of water) and let it dry for two minutes.
Thoroughly wash off all Betadine.
Apply a prescription antibiotic ointment called Bactroban. ... |
ETA we already have the prescr ointment Bactroban (aka Mupirocin).
