Vaginal Breech Birth...

I am 31 weeks and baby is/has been breech pretty much the whole time. She was transverse at my 20 week ultrasound and since then she seemed to be either breech or transverse (transverse is painful, and this being my 4th, I could tell she was breech). I had another ultrasound last week for a study I am in (National Children's Study does an extra 3rd trimester u/s) and yep, she is breech. She also showed me I have been right as she went to transverse and then back to breech during the u/s.

So here's the background. I had a c-section with my 1st (unnecessary - my current OB lists the cause as "physician laziness"), an 8 hour VBAC with my 2nd - 6 pushes, a 2 hour VBAC with my 3rd - 1 push. All were in the 60th-70th percentile... where this baby measures by u/s (inaccurate, yes... but with my history of size, it is believable), and both 2nd and 3rd had about 15" heads.

My OB is not concerned at all about a vaginal breech birth with my delivery history. I was "made for birthing babies" as he puts it. He is trained and experienced with breech births, and prefers them over c-sections (he has a very low c-section rate). The only concern is that the hospital has weight limits, so I may end up being induced around 38 weeks, depending on u/s sizing (which is of course not accurate and he agrees with that 100%, but it isn't his rule, although he will fight it as much as he can to push induction out to at least 38-39 weeks). I have been induced with all so far for various reasons, so I am not concerned about that at all.

I am also trying some positions/exercises to encourage her to turn. But as I have an anterior placenta and the u/s showed a great deal of cord in her facial region, I worry/wonder if she isn't getting caught up in the cord when she starts turning down and thus doesn't get past transverse. I'd rather have a breech birth than a cord around the neck. If she is still breech at 34 weeks (we will check by u/s) I am going to begin treatment with my chiropractor and try acupuncture as well.

I guess what I am looking for are personal experiences and stories about either vaginal breech births or getting babies to turn. I am fairly confident I can do this either way, without a c-section, but can sure use some positive stories!

At 31 weeks I would not worry. Baby has plenty of time to turn. Mine flipped at 36 weeks.

I would be extremely cautious about induction. Inducing at 38 weeks for weight is crazy in general but would be specifically risky for you as a VBAC. It's likely that you would not be ripe enough for any VBAC-okay methods.

No btdt words but I'm in a simillar situation... 37-38 weeks, baby is breech...Frank or complete depending on the time of day and if he's kicking. I've just been risked out of my Birth Center. I've tried chiro, spinning babies, and now a failed ECV.

There are two ob's in my county that will consider a vaginal breech. Both find an epi helps mom dilate better/fuller without the pushing urge. The ob it looks like I'm gong with will often do a scan of mom's pelvis to assure the outlet is large enough. He does not allow the baby to get over a certain weight as determined by u/s. Possible induction if this happens...or it becomes a c-section. So the things your doc is suggesting are
the same mine is.

I don't understand why a hospital has 'protocols' that go against what even ACOG says?!?! Who are they to say what is best for mom and baby? Maybe you could get with who ever is in charge of those protocols and sign an AMA form or something, or just not show up till spontaneous labor? I know that may be hard for your OB to go with, but I'd rather that than an induction. That's just me though. It's great he's supportive of a breech birth though!

Cindy, I am not able to even try ECV because I have an anterior placenta... I think even having that as an option would help me stress less about it. Love hearing that other OBs will do them. I think most VBB are done by midwives, aren't they?

I have 2 friends whose babies turned very late, so I am hoping she turns eventually. I noticed today that twice now she has gotten hiccups after I try the inversion exercises... both times she turned transverse right after the inversion and went back to breech not much later like usual (she has gone transverse without the exercies numerous times) which makes me worry that she is in fact getting caught in the cord when she tries to turn. (I have a friend who has a LOT of experience with cord issues and hiccups can be a sign of cord issues, at least when they get them often)

Thankfully there are no concerns about my pelvis size, as proven by delivering two 15" heads already with minimal effort... and my OB won't do VBB for 1st vaginal deliveries because of all that. If we proceed with a VBB in any way, it will be with an epidural and in the OR, which sucks, but I'd rather that then a c-section! Haha!

I am not too concerned with the induction itself... I have been induced with all 3 so far. Both my VBACs were cervadil inductions (my OB and I both feel it is best for me and it's his first choice for VBACs - my induction that ended in c-section was pitocin induction and it was horrible) and both were very smooth. Derek was 7lbs 2oz at 38 weeks, and I know my OB won't induce for the sake of VBB before 37 weeks.

I have seen plenty of breech videos and what not, read about it and the risks, but dont know anyone who has actually had one. To hear what others thought of the experience would be great.

Oh, and I have thought about not doing a positioning u/s and just 'assuming' baby has turned and showing up in labor. Haha! My OB probably would be on board with that... not that he would have to "lie" but that we just don't do anything extra just because she was breech at my 30 week u/s.

Heck, he didn't do the u/s and I don't even know that it was on the report he got. He got the info from me.
