I would bring the letter to the police department. Even people with mental illness cannot go around threatening people.

I totally agree with PP. If she's threatening you, it's something the police should know about. If something else happens, they need to know the history. If they talk to her, maybe she'll quit.

Me, or any stranger or non-medical person for that matter, really can't make proper assessments about a person's medical condition or what they "might" do, especially on the limited information provided here. While the police may or may not be helpful, the best thing may be to contact next of kin. There could be any number of things that she is suffering from, whether long term or short term, and if you see it as a potential threat to yourself or to her, then someone (like next of kin) should be contacted to intervene if necessary.

We had an elderly woman in our building with long-term schizophrenia. She was fine for years. Then because of age or something else, she started to forget to take her meds. Strange smells started coming out of her apartment and then she was seen wandering around the streets in confusion. Our board contacted her relatives, and they came and moved her out. Her apartment is now up for sale. Mentally ill people can recognize that they are mentally ill, but in moments of true illness, they lose control. If you don't know her next of kin, then you can just nicely ask her for next of kin contact info in the event that there is an emergency...any emergency. We do the same for our neighbors, as we have no family close by.
