I was just in Costa Rica last November with my two mostly unvax'd children, we were not asked about it on entry although we did go via a cruise (not airplane) so I don't know if there is a difference by method of entry. Generally speaking I have never had to show vaccination status for entry as a visitor into ANY country I have been to, and I have traveled a fair amount. However the regulations for immigration are frequently different, and if you are "moving" there different requirements likely apply vs. "visiting".

As for a visit to the US, I do not believe there are vaccine requirements to visit.
